Learning to learn. Please help!

I'm a long time winning poker player and I'm terrible.  I live in Vegas and beat the 1/3 live games for around $30 an hour.  I draw conclusions based on player behavior over small sample sizes, use betting patterns and physical tells.  Yet, I have little concept of placing players on real, accurate ranges, or clearly evaluating how those ranges narrow through multiple streets.  My mathematical knowledge is nearly nonexistent.  I purchased StoxEV long ago, now CREV, yet never used it because my fundamentals simply aren't strong enough to gain real value. 

I am caught in the limbo of technically being a long term winning poker player, yet fundamentally being quite bad.  I would greatly appreciate advice in terms of learning to learn.  Guidance on a road to follow starting at the basics (relatively speaking,) and the application of those skills and a road to progression.
