How would GTO react to an opponent who only 3bets AA and KK??

If a player only 3bets AA and KK would GTO just fold everything it opens or is there still reasons to call??

There are many factors to consider.

The 3b size and the stack sizes are the most important IMO. The continue range selection would also be important.

The smaller the 3b, the more often we can call (just like any situation). You open button to 2bb and BB 3b AA/KK to 6bb, there is more incentive to call vs when the 3b a more "standard" size. There is an effect on both direct odds and also implied odds.

As stacks get deeper, we have more implied odds. Set mining or speculating becomes more attractive since there are more big blinds at stake.

From a Non GTO perspective, there are a myriad of things to consider:

-Am I a blue line or redline player? This is important since red liners typically apply pressure in the right spots and get "big" folds.

-How likely is my speculative hand to get paid off? I would expect to get paid off more often on a T62 board with 22 vs a 764 board with 66.

-How often do I get outdrawn?

-Blocking/Un-blocking considerations. AK/AQ/KQ are horrible calls vs AA/KK where 44 is a much better call (if the situation is right).

You can set up toy games and play around with these spots. Give Player 1 AA/KK only and give Player 2 QQ-22. Put boards out and see how the players interact post flop vs different lines/stack sizes. Change the Player 2's range to QQ-22 and add in 98s-54s (or whatever continues you wish to add) and do the same thing. Toy games can be very enlightening.

Play around with something like Flopzilla where you can assign a range and see how often it "hits". With pairs, they flop sets between 11 and 12% of the time. From flop to turn, you will turn a set roughly 5% of the time and turn to river you will river a set roughly 5% of the time. This will become apparent when you set 44 vs AA. You will see that 44 has @ 20% equity vs AA if we see all 5 cards. You can also put in something like 54s a and set it to see how often you flop 2pr or better and then how often you flop combo draws/high equity draws.
